Patient Information

Booking an appointment

An Advanced Access appointment system is in operation at the surgery for all clinicians. Its purpose is to enable all patients to see a Health Professional within 24hrs. A number of appointments can be pre-booked up to two weeks in advance. All other appointments can be booked on the day. You have the right to see a particular health professional if you wish, but you may have to wait longer for an appointment.

A considerable number of appointments are lost each day due to patients failing to attend without giving prior notice. Please let us know as soon as possible if you are unable to attend an appointment. Patients arriving late may be required to make an alternative appointment.

Registering with the Practice

At present we are not accepting any applications, except from immediate family members of existing patients.

The reason for this course of action is that our practice list size is full.

Please contact the NHS Business Services Centre (01495 332000) who will provide you with a list of GP practices who are accepting new patients in your area.

Home Visits

Housecalls are recognized as an important aspect of modern general practice in the UK. Requests for housecalls will be assessed by the clinical team and should a visit be considered appropriate then a member of the team (Dr, District Nurse, Midwife or Health Visitor) will visit. Please phone, using the main surgery number, before 11.30am on the day you require the housecall.

Telephone Consultations

Telephone Consultations are available every day and are an effective way of ensuring you have access to clinicians without having to attend surgery. Telephone consultations are appropriate for:

  • minor ailments

  • medication reviews

  • sickness certification

We have a highly qualified team of medical professionals. This includes a number of salaried GP's who are permanent members of the medical team plus experienced and skilled nurses, (including two nurse practitioners) who have an enhanced role and can prescribe medication. Our reception team are trained to ask for details of your medical problem to ensure an appointment is made with the most appropriate clinician.

Test Results

These can be obtained by telephoning or calling into either surgery after 9.30am. Where an urgent action is required we will contact you. For this reason it is important that you keep the practice updated of any changes to your address/contact details.

After taking brief details the receptionist will contact the duty doctor. If appropriate and it’s medically possible for you to attend surgery, you will be given an appointment by the doctor.

Should there be any delay or difficulty in contacting the surgery and the patient is unconscious, suffering heavy blood loss, severe breathlessness or chest pain you should dial 999 and request an ambulance.

(between 6.30pm-8am on weekdays plus all day at weekends, Bank holidays and other public holidays.)

Cardiff Local Health Board is responsible for commissioning the Out Of Hours service, which is for patients who need immediate medical attention and cannot wait until their Practice is next open.

If you require immediate medical attention outside of normal surgery hours, please telephone the usual surgery number. A recorded message will give you the phone number for the Out Of Hours service. A trained health professional will answer your call and take some personal details and basic information about your health problem. If you need to be seen, you will be advised which primary care centre to attend.

On arrival at the centre, you will be assessed by a nurse and referred to an appropriate health professional. A home visit will only be made if considered necessary by a health care professional.

Patient Confidentiality

All Clinicians and members of staff at North Cardiff Medical Centre have a duty to respect and maintain patient confidentiality. Furthermore, subject to service provision requirements, NCMC contracts with third party healthcare individuals and organisations such as locum doctors, nurses, clinical agencies and pharmacists who have a professional duty of confidentiality.

If you have any concerns or objections in relation to your information being shared with other healthcare professionals, please contact Mrs Sarah Morris, Practice Manager.

No personal information will be passed on to any other sources without your prior consent.

All personal information held by the Practice is subject to the Data Protection Act (1998). All staff and Clinicians are trained on Caldicott Principles and Guidelines.

Pharmacy Services

 

Pharmacy Services are available from both the main medical centre on Excalibur Drive and at 67 Thornhill Road.  The Pharmacy is operated by Insync Healthcare Pharmacy Ltd.  The Directors of Insync Healthcare Pharmacy Ltd are

  • Dr Haydn Mayo - Managing Director
  • Mrs Sarah Morris - Human Resource Director

 

Cardiff Community Health Council (CHC)

Cardiff Community Health Council has a statutory responsibility for monitoring the quality and range of services within the primary care sector. Members of the CHC met with staff and patients at North Cardiff Medical Centre and discussed a variety of issues e.g. service provision, patient satisfaction, staffing, premises and future developments. Extracts from the CHC report are detailed below:

  • Excellent practice brochure

  • We took the opportunity to speak with patients and these episodes were very positive

  • Patients have access to a wide range of medical staff support

  • Nurse-led clinics free doctors for more specialised work

  • Broad range of special interests of all the medical staff

Patient Participation Group (PPG)

The PPG members are NCMC patients and represent a cross section of the practice population. The group meets quarterly to discuss service delivery issues from a patient perspective and provides valuable feedback and information for the practice.

The Practice was the first in Cardiff to formally establish a PPG (2004). The Group is affiliated to the National Association for Patient Participation (N.A.P.P.) and was established to “promote the relief of sickness and preservation and protection of health for the public benefit by fostering the highest possible standard of primary medical care through the medium of patient participation”.

The group has a number of objectives, which include the following:

  • To act as a conduit for feedback from patients
  • To review elements of current and future service delivery
  • To provide support and advice to assist the Practice to achieve strategic objectives
  • To influence North Ccardiff Medical Centre's quality agenda

The PPG has delivered three successful 'Health Forum' events dealing with issues related to Diabetes, Men's Health and the Health of Young People, a health education seminar delivered in partnership with Llanishen High School.

Individual members have become involved in a variety of practice activities including patient surveys and assisting with training of administrative team members.  The future work programme includes an extension of the 'expert patient programme', developing links with voluntary sector providers and input into planning patient access and services within the new medical centre.
